         ANAHEIM HIGH SCHOOL ATHLETIC CLEARANCE INSTRUCTIONS

To Students and Parents:
 

-Anaheim Athletics will be using homecampus.com for Athletic Clearances for the 2024-2025 school year.  NO paperwork will be submitted to the athletic department.  All paperwork will be uploaded to the homecampus.com website, and will be verified online by the athletic directors. 

Online Athletic Clearance Instructions

In order for a student to participate in the summer programs, they must have their eligibility packet cleared. 

 

-Students must be cleared through the athletic department before participating in tryouts, practices, or games, and must carry a 2.0 GPA.  

-If students are getting a physical from Kaiser, Kaiser must stamp the AUHSD physical form, and may electronically sign their "full activity; cleared for sports" form by the M.D.

ATHLETIC ELIGIBILITY PACKET:

      1. Create account and complete eligibility information on homecampus.com

      2. Print and upload to homecampus.com the completed  ATHLETIC PHYSICAL FORM (click here) , signed by MD or DO

**Students must have the physical form signed by MD or DO

**Students must have valid insurance
